- CareSearchgp App (free)
Smartphone app for GPs to support care of palliative patients living at home or in residential care. It includes terminal prescribing advice.
- palliMEDS
The palliMEDS app familiarises primary care prescribers with eight palliative care medicines that have been endorsed by the Australian and New Zealand Society of Palliative Medicine (ANZSPM) for management of terminal symptoms. The ANZSPM-endorsed medicines can be used to manage common emergent symptoms in home-based palliative care patients in the last few weeks of life, when people often have problems swallowing.
- Primary Health Networks (PHNs)
Primary Health Networks are supporting primary care to improve care delivery and care coordination in the community. There six agreed priorities for targeted work by PHNs and aged care is one of these. Contact your local PHN to find out what resources they have available for aged care and end-of-life care.
