This eLearning course helps clinicians and managers to provide better care for their patients, helping to initiate end-of-life conversations and assessing their palliative care needs.
The content includes:
- initiating conversations about advance care planning with people living with different stages of dementia
- empowering and preparing families to advocate for the person’s end of life care wishes in the future
- identifying people with dementia who would benefit from a structured assessment of their palliative care needs.
