palliAGED resources for Nurses

palliAGED resources for Nurses

These resources have been developed specifically to help the aged care workforce support the palliative care needs of older people. In aged care palliAGED evidence-based resources guide consistent practice across your facility or service. For nurses working in homecare these digital resources also provide point of care support when and where you need it.

palliAGED App

This app helps nurses working across different settings to care for older people with palliative care needs. The app highlights a care framework built around three care processes – advance care planning, palliative care cases conferences and a care plan for the dying person. You can navigate quickly to the three care processes. The online-offline capacity means that the App can be used anywhere in Australia.

Terminal care symptoms and medicines

The palliAGED Symptoms and Medicines section guides recognition of dying and best practice responses to common symptoms in the terminal phase. Management of medications is a major part of this task along with non-pharmacological approaches. Here the emphasis is on guidance for safe and appropriate use of medications.
