To help careworkers in aged care learn about palliative care and what to do, we have combined common patient/client journeys and the palliAGED Tip Sheets.
For each of the older people you are about to meet there is a journey map to download and an online page to prompt you to think about what you could do to help.
These four journeys were developed through research conducted by the Gold Coast Primary Healthcare Network, Palliative Care Queensland, COTA Queensland, and Beacon Strategies. Read about the development of the journey maps.
